The Improvements under this Contract consist of, but are not limited to, MOT (standard, business sign, arrow board, PCMS, signalization, traffic detection, ped LCD, barricade), protections (inlet, sediment), concrete removal, curb/gutter, sidewalk, driveways), asphalt (milling, resurfacing), signing (single-column, sign strip), pavement markings (RPM, detectable, painted, thermoplastic), drainage adjustments (inlet, manhole), signalization (signal cable, prestressed pole, aluminum pole, vehicular auxiliaries, ped signal, ped detector, loop detector, loop assembly, controller), conduit, pull/splice box, electrical (power, wire, disconnect), CIAC, and tree (root/branch pruning), along State Road 5 (S. Federal Hwy) between State Road 5 (US-1/N. Dixie Highway) and 10th Avenue S for 0.773 miles in the Town of Lantana and the City of Lake Worth Beach, Palm Beach County. BUSINESS DEVELOPMENT INITIATIVE -- Bidders must be a "Small Business" as defined by FDOT. Small business size standards can be found at https://www.fdot.gov/equalopportunity/bdi.shtm. Send a completed and notarized affidavit as an attachment to: fdot.bdi@dot.state.fl.us. Bidders must submit the BDI Reference Sheet (Form No. 275-000-02) with the bid unless bidder is prequalified in the project major work type, per Rule 14-22, Florida Administrative Code.
